Client Liability Waiver

This waiver is required before alcohol service is permitted in any Apex Black Car vehicle. It is also required for bachelor/bachelorette parties, prom transportation, concert and stadium bookings, and any reservation involving minors. The signing party accepts full responsibility for the conduct of all passengers. Apex Black Car reserves the right to terminate service without refund if passenger behavior creates a safety or security risk. This waiver does not limit the cardholder’s responsibility for any vehicle damage caused by passengers.

Acknowledgments

Conduct responsibility. I accept full responsibility for the conduct, safety, and behavior of all passengers in the Apex Black Car vehicle for the duration of this booking.

Alcohol service. If alcohol is permitted, I confirm all passengers consuming alcohol are 21 years of age or older. Apex Black Car chauffeurs may refuse alcohol service at their discretion.

No smoking / vaping. Smoking and vaping are strictly prohibited in all Apex Black Car vehicles. Violation may result in immediate service termination without refund and a cleaning fee of up to $500.

Damage liability. I agree to pay for any damage to the vehicle caused by me or any passenger, including stains, breakage, biohazard cleaning (minimum $350), and structural damage.

Service termination. Apex Black Car reserves the right to terminate service at any time without refund if passenger conduct creates a safety risk for the driver or others.

Minors. If any passenger is under 21, I confirm I am the responsible adult and will ensure no alcohol is consumed by minors.
